Privacy Policy
This policy describes what information this website collects, who receives it, and why. It covers https://hbimplants.com only — not the care you receive in the office, which is governed by the practice's Notice of Privacy Practices.

What this site collects
There is one form on this website: the patient referral form, which is intended for referring dentists rather than patients. Submitting it sends:
- The patient's name and phone number
- The referring doctor's name, phone number and the date of referral
- A description of the case and what is being asked of us
No other page on this site collects personal information. Appointment requests are handled by clicking through to our scheduling provider, described below.
Where a referral goes
A submitted referral is checked for automated abuse by Cloudflare Turnstile, then passed to Formspree, a form-handling service, which stores the submission and notifies the practice. Formspree therefore receives everything entered on the form, including patient information.
Referral submissions are retained for 30 days, after which the practice deletes them. The information a referral contains may also be entered into the patient's clinical record, which is kept separately and governed by the practice's Notice of Privacy Practices rather than by this policy.
Automatic collection
Visiting any page also involves these services, each of which receives your IP address and information about your browser:
- Cloudflare — hosts and delivers this site, and logs requests to it.
- Google Analytics — measures which pages are visited and how people arrive.
- OpenAI Ads — measures whether a visit followed one of our advertisements.
- Google Maps — the map on our home page is loaded from Google.
These services set their own cookies and identifiers, governed by their own privacy policies rather than this one.
Booking an appointment
Our scheduling page shows a booking form served by Jarvis Analytics, our scheduling system. The form sits inside our page, but it is delivered from that company's own website: what you enter into it — your name, contact details, and what you tell us about your visit — goes to that service, not through this one. It sets its own cookies and runs its own analytics — including Pendo and Piwik Pro — while you are on that page, governed by its privacy policy rather than this one. Those scripts load on our scheduling page only; no other page on this site loads them.
What we do not do
We do not sell information collected through this website, and we do not use it for advertising to you personally.
